    1. The administrator of your personal data is Curtis Health Caps S.A. with its registered office in Wysogotowo, ul. Batorowska 20, 62-081 Przeźmierowo, email: odo@chc.com.pl.
    2. You can contact the Administrator in matters related to the processing of personal data and the exercise of rights under the GDPR using the above contact details.
    3. Your personal data will be processed for purposes related to reported cases of violation of law, on the basis of: the legal obligation resulting from the provisions of the Act on the protection of persons reporting violations of law (in accordance with art. 6 letter c of the GDPR). Your personal data will be processed for a period of 5 years from the date of receipt of the report. The Administrator ensures the confidentiality of your data in connection with the received report. In connection with this, the data may be made available only to entities authorized to do so under the provisions of law, and entities to which the controller has entrusted the performance of activities related to the recognition of the notification, including external entities providing legal support and compliance services.
    4. You have the right to request access to your personal data, as well as to rectify (correct) it. You also have the right to request the deletion or restriction of processing, as well as to object to the processing, but this is only available if further processing is not necessary for the Controller to fulfill its legal obligation and there are no other overriding legal grounds for processing.
    5. You have the right to lodge a complaint about the processing carried out by the Controller with the President of the UODO (uodo.gov.pl).
    6. Your data will not be made available to a third country or an international organization. Your data will not be subject to profiling or automated decision-making.
    7. Due to the fact that Curtis Health Caps S.A. only recognizes applications signed with first and last name, failure to provide personal data will result in the application not being recognized.
